Requirements for form (4)

Submit the following documents and photocopies must be visible and attested by Notary public & affixed Revenue stamp of Rs. 1 on each paper.

1). Press publication.

2). Attested copy of Death Certificate.

3). Attested copies of C.N.I.C of all heirs.

4). Heirship Certificate and form “B” of NADRA.

5). Attested copy of Allotment Order, Transfer / Mutation latter.(if any)

6). Copies of all paid Challan.

7). Deed if Relinquishments of Rs.100/=

8). Indemnity Bond of Rs.100/=

9). 3 photograph of Allotee.

10). NADRA family registration certificate.
